Surgical Management of Facial Fractures

Craniofacial fractures require immediate assessment and precise skeletal reduction. Unlike other bones in the body, facial bones house vital organs including the eyes, nasal airway, and the dental occlusion mechanism. Improperly aligned fractures can lead to permanent facial asymmetry, double vision, chronic pain, or inability to chew properly.

1. Mandibular (Lower Jaw) & Maxillary Fractures

Mandibular fractures typically occur at the condyle, angle, or parasymphysis. Dr. Saha utilizes Rigid Internal Fixation (RIF) with titanium miniplates and screws. By structurally stabilizing the bone segment, we eliminate the need for traditional Intermaxillary Fixation (IMF)—preventing weeks of having the jaws wired shut, which can lead to joint stiffness, muscle wasting, and poor nutrition.

2. Zygoma & Orbital Socket (Eye) Fractures

The zygomaticomaxillary complex (cheekbone) and orbital floor form the socket supporting the eye. High-impact blows can fracture the thin orbital floor, causing the eye muscles to herniate downward. This results in diplopia (double vision) and enophthalmos (sunken eye). Precise orbital reconstruction using titanium mesh or biocompatible implants restores the socket volume, freeing trapped muscles and correcting vision.

3. Aesthetic Soft Tissue Closures & Nerve Repairs

Facial lacerations are closed using plastic surgery principles, aligning micro-sutures with Langer's lines (natural skin tension lines) to minimize scarring. In deep wounds, branches of the facial nerve or salivary ducts (parotid duct) may be transected. Dr. Saha performs microsurgical nerve anastomosis and duct stenting to restore facial movement and salivary flow.

Trauma Surgery Q&A

Scientific explanations covering jaw fractures, eye socket repairs, and plate placement.

Displaced jaw fractures should ideally be repaired surgically within 7 to 10 days of the injury. During this initial window, the bone segments remain mobile. After 10-14 days, early fibrous union begins, meaning the bone starts healing in the incorrect, displaced position. Operating after this window requires re-fracturing (osteotomy) the bone, which makes restoring the natural dental bite significantly more complex.
No. By utilizing Rigid Internal Fixation (RIF) with titanium plates and screws, the bone fragments are held firmly together under mechanical stability. This allows patients to open their mouth, speak, and eat a soft diet immediately after surgery, completely avoiding the traditional 4-to-6 weeks of painful jaw wiring (Intermaxillary Fixation).
Double vision (diplopia) occurs when the orbital floor or wall fractures, allowing the fat and muscles surrounding the eyeball (specifically the inferior rectus muscle) to herniate downward into the maxillary sinus. This traps the muscle, restricting the eyeball's upward movement. Surgical reconstruction of the orbital wall releases the entrapped tissue and restores normal eye alignment.
No. Titanium is highly biocompatible and osseointegrates with the surrounding bone. It does not cause allergic reactions, degrade, or trigger airport security detectors. The plates are left in place permanently unless they become infected, exposed inside the mouth, or cause discomfort in cold weather, which happens in less than 5% of cases.
