Temporomandibular Joint (TMJ) Pathology

The Temporomandibular Joint (TMJ) connects the lower jaw to the temporal bone of the skull. This complex joint features a sliding cartilage disc that coordinates jaw movement. Overload from jaw clenching (bruxism), trauma, or arthritic wear can displace this disc, causing severe pain, locking, clicking sounds, or total joint fusion.

1. Disc Displacement with/without Reduction

In early TMJ derangement, the disc slips forward when jaw is closed and pops back into place on opening—this is reduction, which creates the audible clicking joint sound. If the disease progresses, the disc remains permanently displaced forward and fails to reduce, acting as a physical wedge that locks the jaw closed (closed lock).

2. TMJ Arthrocentesis & Trigger Botox

For closed locks or severe chronic joint inflammation, Dr. Saha performs **TMJ Arthrocentesis**. Under local anesthesia, two needles are inserted into the joint space to wash it out with saline. This clears inflammatory chemicals, releases vacuum pressure, and mobilizes the disc. Botox injections are also used to relax hyperactive chewing muscles, relieving bruxism.

3. TMJ Ankylosis & Gap Arthroplasty

TMJ Ankylosis is a severe disease where the jaw joint bone fuses to the temporal bone of the skull base, typically due to childhood joint trauma. The patient suffers from near-zero mouth opening. Dr. Saha performs **Gap Arthroplasty**, cutting away a segment of the fused bone to create a gap, and interposing a muscle or cartilage flap to prevent re-fusion and restore opening.

Clicking in the jaw joint is typically caused by "disc displacement with reduction." The cartilage pad (disc) that normally cushions the joint slips forward when the mouth is closed. When you open wide, the condyle slides forward and slips back onto the thick center of the disc, creating a popping or clicking sound (reducing click).
An acute locked jaw (closed lock) occurs when the displaced joint disc fails to reduce and acts as a physical obstruction. It is treated using TMJ Arthrocentesis. Under local anesthesia, the joint is flushed with sterile saline to clear inflammatory proteins, eliminate joint vacuums, and stretch the joint capsule, allowing manual mobilization of the cartilage disc.
TMJ Ankylosis is a severe condition where the lower jawbone fuses to the temporal bone of the skull, typically caused by untreated joint trauma in childhood. Gap Arthroplasty is the surgical cure: the fused bone mass is cut out, creating a gap of at least 15mm. A muscle or cartilage flap is then rotated into this gap to prevent the bones from healing back together, restoring mouth opening.
Botox is injected into the masseter and temporalis muscles (the primary muscles used for chewing). It blocks the nerve signals that cause involuntary muscle contractions, reducing severe jaw clenching and teeth grinding (bruxism). This relaxes the muscles, decreases joint loading, and resolves myofascial TMJ pain without altering speech or normal chewing.
