Medical Guide

Advanced Dental Implantology

When teeth are lost due to decay, trauma, or gum disease, the surrounding alveolar jawbone quickly resorbs (melts away), leading to facial collapse and adjacent tooth shifting. Dental implants serve as artificial tooth roots, stimulating the bone and preventing resorption while providing a rock-solid foundation for dental crowns and bridges.

1. Full Arch Rehabilitation (All-on-4 & All-on-6)

For patients missing all teeth in an arch, All-on-4 or All-on-6 protocols offer a permanent alternative to removable dentures. By tilting the posterior implants at a 45-degree angle, we maximize contact with dense bone and avoid critical anatomical structures like the sinus cavity. A customized, fixed prosthetic bridge is loaded onto these implants, restoring natural bite force.

2. Zygomatic & Pterygoid Implants

In cases of severe maxillary bone resorption where sinus lifts are contraindicated or have failed, Dr. Saha utilizes Zygomatic Implants. These extra-long implants anchor directly into the dense zygoma (cheekbone), which does not resorb like the jawbone. Similarly, pterygoid implants leverage the dense sphenoid bone in the back of the mouth, allowing immediate loading without the need for extensive, multi-stage bone grafting.

3. Sinus Lift & Alveolar Ridge Grafting

To place standard dental implants, a minimum height and width of bone is required. In the upper molar regions, the sinus floor is often too low. Dr. Saha performs a **sinus lift**, gently pushing the sinus membrane upward and packing autogenous bone or biocompatible bone substitutes underneath. Ridge splitting and bone block grafting are also utilized to widen thin, atrophied alveolar ridges.

Implantology Q&A

Scientific explanations covering dental implants, bone grafting, and zygomatic techniques.

Zygomatic implants are longer than standard implants (up to 55mm) and are placed at an angle, anchoring directly into the dense zygomatic bone (cheekbone) instead of the maxilla (jawbone). The cheekbone does not resorb like the jawbone when teeth are missing. This allows patients with severe upper jawbone loss to receive a fixed set of teeth without undergoing painful, months-long bone grafts.
Osseointegration is the process where living bone cells grow and bond directly onto the microscopic titanium implant surface. In the lower jaw, which is naturally denser, this process takes about 3 months. In the softer bone of the upper jaw, it takes 4 to 6 months before the implant can support the heavy chewing forces of the final prosthetic teeth.
The sinuses (air cavities) lie directly above the upper molar teeth. When these teeth are lost, the sinus cavity expands downward while the jawbone resorbs upward, leaving a very thin layer of bone (sometimes less than 2mm). A sinus lift surgery gently raises the sinus floor membrane and inserts bone graft material beneath it, creating enough solid bone height to anchor dental implants.
Dental implants have an exceptionally high success rate (95-98%). Failure, when it occurs, is typically due to infection (peri-implantitis) or poor bone healing. Uncontrolled diabetes and smoking significantly increase failure rates by reducing blood flow to the bone. Successful prevention relies on strict clinical hygiene during placement, smoking cessation, and meticulous daily brushing and flossing postoperatively.
