Surgical Philosophy in Facial Aesthetics

Facial plastic surgery is a unique specialty that sits at the intersection of aesthetic refinement and functional reconstruction. In the facial region, cosmetic improvements must never compromise primary physiology—specifically, the nasal airway, eyelid blink mechanism, and jaw movements. A thorough understanding of underlying craniofacial anatomy is essential to achieve balanced, natural outcomes.

1. Structural Rhinoplasty & Septal Deviations

Rhinoplasty is not merely about reshaping the nasal bridge. The nasal septum and the internal/external nasal valves form the narrowest segments of the human airway. A deviated septum or weak cartilage can lead to internal nasal valve collapse. Dr. Saha utilizes structural autologous grafts (e.g. spreader grafts, septal extension grafts) to structurally support the airway while refining the external aesthetic profile.

2. Facial Scar Revision & Contracture Release

Facial scars resulting from trauma or prior surgeries can become thick, raised (hypertrophic), or pull tightly on surrounding features (scar contracture). Revision is performed using geometric repositioning techniques like Z-plasty or W-plasty. This changes the direction of the scar to lie parallel to relaxed skin tension lines, breaking up visual straight lines. Note on Timing: Scars undergo dynamic remodeling and must be allowed to fully mature for 6 to 12 months before surgical revision is attempted.

3. Eyelid Surgery (Blepharoplasty) & Facial Rejuvenation

Excess eyelid skin (dermatochalasis) or fat herniation can cause a tired appearance and obstruct the upper visual fields. Blepharoplasty removes excess skin and fat while preserving the eye's natural shape and protective blink mechanism. In reconstructive cases, Dr. Saha corrects eyelid malpositions such as ectropion (eyelid turning outward) or entropion (eyelid turning inward) to protect corneal health.

Facial Plastic Q&A

Scientific explanations covering septorhinoplasty, Cottle maneuvers, scar timing, and keloid therapy.

The Cottle maneuver is a simple diagnostic test where the cheek is gently pulled laterally (outward) next to the nostril. If this action immediately improves nasal airflow on that side, it indicates internal nasal valve collapse (narrowing of the airway passage). This indicates that standard rhinoplasty or septoplasty must incorporate structural cartilage grafts (like spreader grafts) to reinforce the valve and prevent collapse.
Scars undergo a complex maturation process that takes 6 to 12 months. Early scars often appear red, raised, and firm but may naturally soften and flatten over time. Revising a scar before it has fully matured can result in poor healing, wound dehiscence, or recurrence of thick tissue. Therefore, scar revision is typically planned only after the scar is at least 6 to 12 months old.
Cosmetic rhinoplasty focuses solely on the external appearance of the nose (e.g., removing a dorsal hump, refining the nasal tip). Septorhinoplasty addresses both the external shape and the internal septum (the partition dividing the nostrils). If the septum is deviated, it blocks the airway. A septorhinoplasty corrects the deviation to restore breathing while simultaneously refining the nose's external contour.
Hypertrophic scars remain within the boundary of the original wound, while keloids grow beyond the margins and are highly prone to recurrence. Management involves surgical excision using micro-sutures along natural skin tension lines, often combined with post-operative therapies such as localized corticosteroid injections, silicone gel sheeting, or pressure dressings to suppress collagen overproduction.
